The Jockey Club CADENZA Hub located in Tai Po, New Territories, is one of the innovative community projects of “CADENZA: A Jockey Club Initiative for Seniors”, funded by The Hong Kong Jockey Club Charities Trust, to meet the various health and social service needs of increasing ageing population. It aims at improving quality of care and quality of life for older persons in Hong Kong.

The Jockey Club CADENZA Hub, managed by The Chinese University of Hong Kong and supported by an interdisciplinary professional team, is a one-stop fully integrated and self financed health and social care centre. The Hub adopts a client-centred case management approach in offering primary health and social care. To help older persons live healthily and actively, the Hub provides a wide array of wellness, lifestyle modification and health maintenance programmes, as well as other co-located services such as traditional Chinese medicine, optometry, etc.
